- Physical address of interpreter's base or service area (2nd Line).
- City (Optional)
- City where interpreter works or is based.
- State (Optional)
- State where interpreter works or is based.
- Zip (Mandatory)
- Zipcode of interpreter's base (used for service area searches).
- Email (Optional)
- Email address client should use to contact interpreter.

- Web address of interpreter (or agency).

- Is trained in Sign Language interpreting.
